The Bellarine Community Health Palliative Care nurses work with individuals, their families and a multidisciplinary team to coordinate and provide high quality end of life care.


Palliative care support may be provided in the home (including residential aged care facilities), hospitals and in hospices.

Bereavement Program

Coping with loss can dramatically affect your physical, emotional, social or financial wellbeing. 

Bereavement Support services are provided through the Palliative Care Nurse, counselling and volunteers.
